ForHealth Group is seeking a Registered or Enrolled Nurse to join Mount Druitt Medical Centre, supporting GPs in chronic condition management and preventative healthcare. You will work with a supportive team focused on patient outcomes and culture.

Qualifikationen

• Current AHPRA registration as Enrolled Nurse (EN) or Registered Nurse (RN).
• Experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.

Aufgaben

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.
