Job summary

ForHealth Group is seeking a Registered or Enrolled Nurse for our Maroubra Medical Centre, to support GPs and patients through chronic condition management and preventative care.

You'll be part of a supportive clinical team, with opportunities to grow your career through training and internal progression, and flexible part-time or full-time arrangements.

Qualifications

• Current AHPRA registration as EN or RN.
• Experience in chronic condition management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
• Conduct health assessments and promote preventative care
• Collaborate with GPs and multidisciplinary teams, including referral coordination
• Manage administrative tasks related to CCM appointments
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women's health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About the Role
