Job summary

Concentric in Carrum Downs is seeking a Positive Behaviour Support Practitioner to join a multidisciplinary team. You will support adolescents and adults with varied diagnoses, improving mental health and wellbeing through evidence-based plans and collaborative care.

You will deliver assessments, develop strategies for behaviours of concern, and contribute to reports and goals within NDIS plans, with mentoring and flexible work options to support growth.

Qualifications

  • NDIS registration at core or proficient level.
  • APHR A registration.
  • Valid Australian driver’s licence.

Responsibilities

  • Evaluate, develop and deliver evidence-based positive behaviour support plans and interventions.
  • Conduct assessments for participants of all ages and abilities.
  • Develop, deliver and monitor interventions for behaviour management.
  • Collaborate within a multi-disciplinary team.
  • Complete reports within NDIS plans.

About Your New Role

Become part of a team who are committed to assisting a range of adolescents and adults with various diagnosis and presentations. Grow your career within a collaborative and supportive multidisciplinary team and assist clients in improving their overall mental health and well-being.

  • Evaluating, developing and delivering evidence-based positive behaviour support plans and interventions
  • Conducting a variety of assessments for participants of all ages and all disabilities
  • Develop, deliver and monitor interventions and strategies for the management of behaviours of concern
  • Work collaboratively within a multi-disciplinary team
  • Complete reports and work towards goals within the NDIS plans

Essential Qualifications and Experience:
  • Registered with NDIS at core or proficient level.
