Job summary

Cairns and Hinterland Hospital and Health Service in Cairns is seeking a Physiotherapist to deliver Transition Care Program services, including assessment, intervention and case management for frail elderly clients.

You will collaborate with families, health professionals and researchers, contributing to program development and quality improvement within a supportive regional healthcare setting.

Responsibilities

• Provide competent comprehensive physiotherapy assessment, intervention and case management of clients referred to the Transition Care Program including liaison with key stakeholders.
• Demonstrate communication and teamwork skills, by ensuring effective liaison with Transition Care Program clients, their families, other health professionals, supervisors, students and researchers, as required.
• Deliver evidenced based physiotherapy programs to a frail elderly client load.
• Participate in the development of the Transition Care Program and Physiotherapy quality activities, clinical practices, procedures and protocols.
