Queensland Health is seeking a Physiotherapist to join the Transition Care Program in Cairns. You will provide an effective and efficient physiotherapy service to older people, and their carers and families, discharged from acute and sub-acute care settings into the Transition Care Program.

The Transition Care Program are seeking a Physiotherapist to join them. In this role you will provide an effective and efficient physiotherapy service to older people, and their carers and families, who have been discharged from acute and sub-acute care settings and accepted into the Transition Care Program. This includes working collaboratively within a multidisciplinary team to case manage the wider needs of allocated clients.
