Job summary

The Queensland Government’s Cairns and Hinterland Hospital and Health Service is seeking a skilled physiotherapist to join the Transition Care Program. You will provide comprehensive assessments, develop intervention plans and coordinate case management for referred clients.

You will liaise with clients, families and other health professionals, deliver evidence-based programs for frail elderly clients and contribute to clinical practice improvements and protocols.

Qualifications

• Qualified physiotherapist registered to practice in Australia.
• Experience with Transition Care Program or frail elderly clients is desirable.
• Ability to communicate effectively with clients, families and health professionals.

Responsibilities

• Provide comprehensive physiotherapy assessment, intervention and case management for Transition Care Program clients.
• Liaise with clients, families, health professionals and supervisors to coordinate care.
• Deliver evidence-based physiotherapy programs for frail elderly clients and contribute to program development.
