Catholic Healthcare Limited is seeking a Physiotherapist for community aged care in Sydney, offering a permanent full-time role with client visits across South West & Inner West Sydney. Responsibilities

• Conduct comprehensive assessments of clients’ physical and functional abilities.
• Develop and deliver individualised or group treatment plans.
• Implement evidence-based physiotherapy interventions for mobility and pain management.
• Regularly assess and report client progress to care management.
• Maintain accurate records, exercise sheets, and progress notes.
• Provide recommendations on home safety and equipment as needed.
• Collaborate with interdisciplinary teams to deliver holistic care.
• Educate clients and families on preventive measures and exercises.
• Participate in team meetings and training sessions.

• Pay rate – up to $57.00 per hour (based on experience) + superannuation + $1.11 per km

This is an exciting opportunity for a Physiotherapist looking for a varied role in Community Aged Care on a permanent Full-time basis. This role involves assessing, planning and delivering individualized/group physiotherapy services and activities to improve the well-being of clients in their own homes as well as running group sessions across our Wellness Hubs.

Day to day this role looks like:

• Conducting comprehensive assessments of our clients physical and functional abilities to gain an understanding of their needs
• Developing and delivering individualised or group treatment plans based on clients’ needs, goals, and preferences
• Implementing evidence-based physiotherapy interventions to address mobility issues, pain management, and other age-related conditions
• Regularly assessing and reporting on the physical and psychosocial capabilities of clients to care management team
• Maintaining, preparing and supporting precise records, exercise sheets, assessment reports and progress notes that accurately reflect clients’ physical function.
• Making recommendations in relation to exercise programs, mobility equipment, exercise equipment, safety at home and referring to other disciplines as required.
• Upholding ethical and professional standards in accordance with regulatory guidelines.
• Collaborating with interdisciplinary teams, including care advisors, occupational therapists, fitness instructors, allied health assistants, and other healthcare professionals, to provide holistic care.
• Educating clients and families on preventive measures and exercises to improve and maintain mobility.
• Participating in team meetings, case conferences, and training sessions to contribute to the continuous improvement of care delivery
• Engaging with team members, clients and client’s families in a positive and respectful manner, welcoming a culturally diverse team
• Building relationships with clients and their families, understanding the challenges they face in the ever-changing landscape of aged care.

To be successful in this role you’ll need:

• Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration
• Excellent interpersonal,verbal,and written communication skills.
• Ability to manage all aspects of compliance
• Demonstrated experience with multidisciplinary team working.
• Effective time management skills to ensure patient intervention, reports and outcomes are achieved in a timely manner and the ability to effectively prioritise and organise own work/caseload.
• Demonstrated computer literacy
• Demonstrated knowledge and experience of health, aged and/or disabilityservices
• Current Driver’s licence and registered and insured motor vehicle
