Carrington Centennial Care is seeking a skilled Physiotherapist to support independence and mobility for older people in our not-for-profit aged and community care services. You will tailor your caseload across onsite and community clients, working with a multidisciplinary team to deliver person-centred care and evidence-based practices.

Qualifications

• AHPRA general registration required; new grads welcome.
• Excellent interpersonal and written communication.
• Ability to work independently and in a team.

• Strong clinical reasoning and results-oriented.

• WH&S and manual handling knowledge.
• Willingness to travel to community clients.
• NDIS Worker Check clearance and pre-employment assessment.

Responsibilities

• Assess residents' physical conditions and functional abilities.
• Develop and evaluate care plans and functional health assessments.
• Create evidence-based treatment plans and coordinate reablement programs.
• Implement falls prevention strategies.
• Provide community-based physiotherapy extending to broader community.
• Manage private physiotherapy services and personalised care.
• Conduct pre-employment assessments for new staff.
• Collaborate with multidisciplinary teams for care planning.
