Job summary

Carrington Centennial Care is seeking a skilled Physiotherapist to join our allied health team in a full-time role based in NSW. You will assess residents, develop care plans, and implement evidence-based physiotherapy programs to improve mobility and independence for older adults.

You will work across onsite and community settings, collaborating with a multi-disciplinary team to deliver person-centered care, falls prevention, and reablement.

Qualifications

  • AHPRA registration (general) required; new grads welcome.
  • Excellent interpersonal, oral and written communication skills.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Willingness to travel to clients in the community.
  • NDIS Worker Check Clearance required during recruitment.

Responsibilities

  • Assess residents' physical conditions and functional abilities.
  • Develop and evaluate care plans and functional health assessments.
  • Coordinate reablement programs and therapy plans.
  • Implement falls prevention strategies and provide community-based care.
  • Coordinate private physiotherapy services as needed.
  • Conduct pre-employment assessments of new staff; recommend accommodations.
  • Collaborate with multidisciplinary teams for care planning.

The Opportunity

We have an opportunity for a skilled and empathetic Physiotherapist to join our dynamic and expanding team in a Full Time capacity (8:00am-4:00pm), enabling wellbeing and quality of life for older people in care and living independently. Help clients achieve improved mobility, pain relief, and a better quality of life, and enjoy growing in a supportive team focused on service that is person-centred, best practice and continuously improving.

Key Responsibilities:

  • Conducting assessments of residents' physical conditions and functional abilities
  • Developing and evaluating resident care plans and formulating functional health assessments for new residents
  • Developing evidence-based treatment plans and coordinating client reablement programs and therapy plans
  • Implementation and management of falls prevention strategiesPracticing in community care, extending physiotherapy expertise beyond our facilities to support the broader community
  • Managing private physiotherapy services, offering specialised physiotherapy care to individuals seeking personalised treatment
  • Conducting pre-employment assessments for new staff to evaluate their physical capabilities and recommending adjustments or accommodations as needed
  • Collaborating with multidisciplinary teams to ensure comprehensive care planning, including case conferences and care planning meetings
