Job summary

CommonSpirit Health at Home is seeking a skilled Physical Therapist to provide in-home rehabilitation services, promoting recovery and improved quality of life for patients in their own homes.

You will conduct evaluations, implement exercises and manual therapy, educate families, and document progress while adhering to professional standards and agency policies.

This role requires collaboration with an interdisciplinary team to address each patient’s goals and ensure safe, effective care.

Qualifications

• Bachelor’s degree from an accredited school approved by the American Physical Therapy Association.
• Active registered PT license in the state where you practice.
• Must obtain and maintain CPR/BLS Certification.
• Driver's license required.
• Compliance with federal, state and local laws and regulations.

Responsibilities

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ function and mobility.
• Educate patients and families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.

Skills

CPR/BLS Certification

Active PT License

One-year PT experience preferred

Education

Bachelor’s degree in Physical Therapy (ACPTA accredited)

Job Summary and Responsibilities

We are seeking a skilled and compassionate Physical Therapist to join the Southeast Al Health at Home team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals. The focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most "functional" place to make an impact on this!

Responsibilities:

Job Requirements

• One-year physical therapy experience in teaching or clinical setting is preferred, but not required.

Pay Range

$43.10 - $64.11/hour
