Job summary

New Solutions in New South Wales is seeking a dedicated Physical Therapist for home health to deliver patient-centered care in patients' homes. This full-time role focuses on assessment, treatment planning, and rehabilitation to improve mobility and daily function.

The ideal candidate has at least nine months of PT experience, a CAPTE-approved master's degree, and current CPR certification. Community/home health experience is preferred; benefits include health, retirement, and PTO.

Qualifications

• Minimum nine months experience as a physical therapist.
• Master's degree in Physical Therapy from a CAPTE-accredited program.
• CPR certification required to practice in a clinical role.
• Home health experience is preferred.

Responsibilities

• Perform comprehensive physical assessments and analyze medical histories.
• Create and implement tailored treatment plans addressing rehabilitation goals.
• Conduct therapeutic exercises to improve mobility and function.
• Educate patients and families on injury prevention and recovery.
• Monitor progress and adjust plans to meet rehabilitation goals.
• Collaborate with multidisciplinary healthcare team to ensure holistic care.

Position Responsibilities:

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
• Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs.
• Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality.
• Educate patients and their families about injury prevention, wellness strategies, and the recovery journey.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals.
• Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience
• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program
• Must possess and maintain valid CPR certification while employed in a clinical role
