Claro Disability Services is seeking a part-time permanent disability support worker in Adelaide to help people with complex needs achieve independence. You’ll provide person-centered planning, mobility and daily tasks, medical supports, and community participation.

You’ll collaborate with families, medical professionals, and a clinical team to tailor supports to each client’s goals, values, and culture, while building trusting relationships and ensuring safety.

Qualifications

• Person-centered planning and communication with clients and families.
• Ability to adapt to changing client needs and routines.
• Strong teamwork with families, medical professionals, and support staff.

Responsabilités

• Person-centered planning: chat with client and family to understand goals.
• Complex physical care: assist mobility, transfers, positioning, daily tasks.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: provide meaningful activities and access.
• Autonomy and advocacy: support decision-making and independence.
• Cultural humility: respect backgrounds and beliefs.
