Job summary

CatholicCare in Taree is seeking an experienced Clinical Coordinator to support people with severe and complex mental illness through coordinated, person-centred clinical care. This permanent part-time role runs three days per week.

You will provide trauma-informed, recovery-focused care based on a plan developed with the client, their GP, psychiatrist or other supports. You’ll work with a Peer Support Worker to keep clients empowered and connected.

Qualifications

• Evidence of independent practice history and skill in meeting the range of demands of complex caseloads.
• Good working knowledge of mental health, local networks, legislation and practices.
• Excellent skills in implementing strategies that promote emotional and developmental well-being in people who are experiencing ongoing, complex mental health disorders.
• Demonstrated ability to build relationships and collaborate with external stakeholders to provide a holistic approach to support for clients.
• Demonstrated communication skills and the ability to write detailed and informed notes.

Responsibilities

• Provide coordinated clinical care for people with severe and complex mental illness.
• Develop and deliverGP Mental Health treatment plans in collaboration with a GP or Psychiatrist.
• Collaborate with a Peer Support Worker to deliver person-centred holistic support to clients.
• Work with GPs, Psychiatrists and allied health professionals, and families to support clients on their mental health journey.
