Job summary

CatholicCare Social Services Hunter Manning is seeking an experienced Clinical Coordinator to join our Mental Health team in Taree, NSW. This permanent part-time role (three days per week) focuses on coordinated, person-centred clinical care for individuals with severe and complex mental illness.

You will provide trauma-informed, recovery-focused support and work with clients, their GP, psychiatrist and other supports to deliver holistic care.

Qualifications

• Registered mental health professional qualification and mental health experience.
• Ability to practice independently and meet complex caseload demands.
• Strong communication skills and ability to document notes clearly.
• Experience collaborating with GPs, psychiatrists and other health professionals.

Responsibilities

• Provide coordinated clinical care for people with severe and complex mental illness.
• Develop and implement care plans in collaboration with clients and healthcare providers.
• Collaborate with GPs, psychiatrists and allied health professionals to support clients.
• Deliver trauma-informed and recovery-focused support within a person-centred framework.
• Support clients with autonomy and meaningful community connections.
