Communify is seeking passionate Mental Health Practitioners to join the MHCP Pilot at the Redcliffe Medicare Mental Health Centre in Brisbane. You will work in a multidisciplinary team to provide coordinated, recovery-focused care for people with complex needs.

Responsibilities include care planning, delivering psychosocial interventions, outreach within GP practices, and ensuring quality documentation. Applicants should have relevant qualifications and strong collaboration skills.

Competenze

• Qualification in Social Work, Psychology, Occupational Therapy, Mental Health Nursing, Behavioural Science, Counselling, Human Services or related discipline.
• Experience supporting people with severe and complex mental health concerns.
• Knowledge of trauma-informed, recovery-oriented and strengths-based practice.
• Experience assessments, care coordination and psychosocial interventions.
• Excellent communication and relationship-building skills.
• Confidence working with General Practice, health services and community organisations.
• Strong organisation and experience using electronic client management systems.
