Northern Health invites applications for a Mental Health Clinician in the Enhanced Clinical Stream at Hotham Street Community Mental Health in Preston. The role supports adults with complex needs within a multidisciplinary team and involves intensive treatment, outreach, and coordination of care.

The successful candidate will provide assessments, treatment planning, and collaborative care, with supervision and professional development opportunities in a supportive environment.

Qualifikationen

• Current registration with AHPRA (where applicable) or eligibility for membership with the relevant professional body.
• Current Victorian Driver Licence and ongoing ability to use this form of transport.

Aufgaben

• Provide comprehensive mental health assessments, risk assessments and recovery-oriented treatment planning.
• Deliver intensive case management, care coordination and assertive outreach for consumers with complex mental health needs.
• Develop and maintain therapeutic relationships with consumers, carers and families that promote engagement, recovery and wellbeing.
• Work collaboratively with multidisciplinary teams, general practitioners, NDIS providers, housing, AOD and other community support services.
• Deliver discipline-specific evidence-based interventions that support consumer goals and recovery.
• Participate in clinical reviews, case conferences, care planning and multidisciplinary treatment discussions.
• Maintain accurate and timely clinical documentation in accordance with organisational and legislative requirements.
• Participate in clinical supervision, professional development and service improvement activities.
• Contribute to student education, staff supervision and portfolio responsibilities as appropriate.
