Job summary

Western Health in Melbourne's west seeks a Mental Health Clinician to join the Intensive Community Treatment team delivering home-based care for people aged 65 and over with acute mental illness. You will perform assessments, create treatment plans and monitor recovery in a recovery-focused, person-centred approach.

Join a multidisciplinary team, provide compassionate, evidence-based care, and support consumers, families and carers with coordination and documentation.

Qualifications

• Demonstrated skill and experience in community-based management of services and assessment and treatment of older adults.
• Ability to develop and implement individualized treatment plans.
• Knowledge of legislation relevant to Older Adults' Mental Health and in particular the Mental Health and Wellbeing Act (2022).

Responsibilities

• Deliver evidenced based treatment and interventions that support consumers with complex mental health related needs and their families/carers.
• Promotion of Mental Health evidence-based practice.
• Successful completion of required mandatory training activities, including training related to the National Standards.
• Compliance of clinical governance, legislative and documentation requirements.
