Latrobe Regional Health in Gippsland, Victoria, is seeking a HIP Care Coordinator on a 0.5 EFT ongoing basis. You will assess needs, coordinate services and manage a caseload through community-based programs, primarily within HARP, with exposure to TCP and RIR.

Role focuses on preventing avoidable hospital presentations, supporting chronic disease management and coordinating care across acute, community and aged care services. A supportive, diverse workplace is offered.

Qualificações

• Current Division 1 RN registration with AHPRA.
• Minimum five years post-graduate experience in an acute healthcare setting.
• Current Victorian Driver Licence.
• Experience in patient assessment and management of complex health needs.
• Ability to work effectively within an interdisciplinary team to deliver person-centred care.

Responsabilidades

• Assess, coordinate and manage a caseload of HIP clients to improve outcomes.
• Develop and evaluate personalised care plans for chronic conditions.
• Coordinate care across acute, community, and aged care services.
• Provide clinical assessment, education and support to clients and families.
• Work with a multidisciplinary team to deliver evidence-based care.
• Contribute to quality improvement and service development.
