Job summary

Latrobe Regional Health invites applications for the HIP Care Coordinator role within the Health Independence Programs (HIP) team. The position is part time (0.5 EFT) on an ongoing basis, based in Gippsland, focusing on reducing hospital presentations and coordinating community care for clients with chronic conditions.

Qualifications

• Current Division 1 Registered Nurse with AHPRA.
• Minimum five years post-graduate experience in an acute healthcare setting.

Responsibilities

• Assess, coordinate and manage a caseload of clients in Health Independence Programs.
• Develop, implement and evaluate individualised care plans for patients with chronic conditions.
• Coordinate care across acute, community, specialist and aged care services.
• Provide clinical assessment, education and support to clients and families.
• Work collaboratively within an interdisciplinary team to deliver evidence-based care.
• Contribute to quality improvement and safe, person-centred care.
