Hunter New England Local Health District is seeking a Registered Nurse to support older people transitioning from hospital to home across Taree, Forster or Gloucester. The role is permanent part-time, 20 hours per week, with a salary of $41.22–$57.88 per hour plus 12% superannuation and benefits.

You will assess patients, develop plans, and coordinate care with families and a multidisciplinary team to promote independence and safe home-based recovery.

Qualificações

• Registered Nurse with AHPRA registration
• Strong clinical assessment and care coordination skills
• Effective communication, organisation and time management
• Ability to work independently, adapt to changing needs and prioritize demands
• Genuine passion for helping older people regain independence
• Experience in community nursing, rehabilitation or transitional care is advantageous

Responsabilidades

• Complete patient assessments and coordinate referrals from hospital.
• Develop care plans to support recovery, mobility and independence.
• Manage patient care and monitor progress throughout the program.
• Collaborate with patients, families and carers to set meaningful goals.
• Coordinate ongoing support and safe transitions from the program.
• Work with nursing, allied health and community support teams.
