Job summary

SA Health is seeking a Clinical Program Manager to lead integrated care initiatives within the Primary and Urgent Care Integration team. The role leverages clinical expertise to drive reform, redesign and commissioning across Local Health Networks and SA Ambulance Services.

Collaboration across the department and partners is essential to deliver patient-centred improvements. Applicants must be eligible for Nurse registration with NMBA and hold or be eligible for a current practicing certificate,

Qualifications

• Clinical knowledge to facilitate reform and redesign of services.
• Experience with integrated care initiatives and program delivery.
• Ability to work with multiple stakeholders across LHN and SA Ambulance.

Responsibilities

• Use clinical knowledge to facilitate reform, redesign and commissioning of integrated care initiatives.
• Collaborate with Local Health Networks and SA Ambulance Services to implement continuous improvement processes.
• Support development and delivery of integrated care initiatives with diverse project partners and stakeholders.
• Contribute to the Integrated Care business plan and leadership team.
