SA Health is seeking a Clinical Program Manager to lead reform and commissioning of integrated care services in the Primary and Urgent Care Integration portfolio. You will work closely with PCCs, Local Health Networks and SA Ambulance Service to enable scalable, consistent practice and improved patient-centred outcomes.

The role requires strong clinical knowledge, leadership and collaboration, with alignment to the Integrated Care Directorate and associated teams within SA Health.

Qualifications

• Clinical knowledge and experience to facilitate reform and redesign of services.
• Leadership and management capabilities to support integrated care initiatives.
• Good collaboration with stakeholders to enable patient-centred care delivery.

Responsabilités

• Facilitate reform, redesign and commissioning of services and programs required to contribute to integrated care initiatives.
• Collaborate with Clinical Project Lead Priority Care Centres, Local Health Networks and SA Ambulance Service to implement continuous improvement processes at scale.
• Support delivery of system and service process improvements for patient-centred responses in partnership with diverse stakeholders.
• Contribute leadership and clinical knowledge to the Integrated Care Directorate and the Department for Health and Wellbeing.
