Job summary

NCN Health in Numurkah is seeking a Carer Support Coordinator (Fixed Term - Part Time) to coordinate flexible respite services for unpaid carers. The role works within the Community Aged Care team and supports carers of people with care needs.

Training and a multidisciplinary environment are provided. The ideal candidate holds a health-related tertiary qualification and experience in assessment and care planning, with strong interpersonal and organisational skills.

Qualifications

• Vaccination against Covid-19 (recommended).
• Tertiary qualification in a health related discipline, including Nursing, Disability, Welfare and/or Community Services practice including registration with a relevant regulatory body (e.g., AHPRA) if applicable.
• Demonstrated experience in assessment, care planning and coordination of services for individuals and families.

Responsibilities

• Provide information, education and resources to carers.
• Plan and coordinate flexible respite services to support local carers.
• Assess needs and develop individualised supports and service plans.
