NCN Health is seeking an enthusiastic Carer Support Coordinator to join our team in Numurkah. Funded under the Department of Families, Fairness and Housing's Support for Carers Program, this rewarding role is a fantastic opportunity to make a meaningful impact in our community.
The Carer Support Coordinator is responsible for the provision of information, education and resources, and supports the planning and coordination of flexible respite services to support local carers who provide unpaid care to a family member, friend or person who is frail aged, has dementia, a disability, mental illness or a medical condition.
The Carer Support Coordinator will assess the needs of the individual carers and work with them to plan and coordinate the provision of supports and services that meet their individual circumstances.
The NCN Health Carer Support Program provides person centred care and support via provision of flexible respite and support services for unpaid carers of people with care needs.Services aim to assist in meeting the diverse and individual needs of carers, promoting improved health and wellbeing of carers, and those for whom they care.
The Carer Support Coordinator is an integral part of the multidisciplinary Community Aged Care team.
