Job summary

UnitingCare Children, Young People and Families – Macleay Island QLD is seeking a dedicated Care Partner for BlueCare Community Care. This full‑time, max‑term role involves leading care planning, coordinating services, and working with clinical teams to deliver person‑centred support across Redland Bay to Victoria Point and surrounding areas.

You will travel regularly to Macleay and Russell Islands, manage budgets, review care plans, and help clients navigate funding streams.

Qualifications

• Certificate IV in Health or Community Service or equivalent is essential (as stated).
• Current Australian Driver’s Licence is essential.
• A roadworthy and insured vehicle is essential for work travel.
• Demonstrated understanding of client-centred, community-based care delivery.
• Ability to liaise with clinical and care teams to coordinate care and exercise professional judgment.

Responsibilities

• Develop and review care plans with the client and care team.
• Manage budgets and facilitate multidisciplinary care activities.
• Monitor and respond to changing needs of complex clients.
• Continuously review and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and access resources.
