Job summary

Connect2Care in Sydney is seeking an Occupational Therapist to join a multidisciplinary team delivering evidence-based services across community, telehealth, and centre-based care. This mobile role offers flexibility to work from home or hubs when not with clients.

The New Graduate Program provides a 12-month induction with weekly supervision, shadowing opportunities, core skills training, fortnightly mentoring, mental health check-ins, and access to senior clinician support.

Qualifications

• OT qualification and registration with AHPRA.
• Current WWCC, NDIS Worker Screening and Police Check.
• Valid driver’s licence and reliable vehicle.

Responsibilities

• Assess and treat clients in paediatrics and adults within community, telehealth and hubs.
• Support goals and functional outcomes aligned with NDIS plans.
• Provide AT assessments and recommendations.
• Write clear, timely reports and manage a varied caseload.
• Collaborate with families, referrers and stakeholders.
• Engage in supervision and ongoing professional development.

About the Role

As an OT at Connect2Care, you’ll be part of a multidisciplinary team providing evidence-based services through a blend of community, telehealth, and centre-based care. This mobile role allows flexibility to work from home or our hubs when not with clients.

Key responsibilities:

• Assess and treat clients with diverse needs (paediatrics + adults)
• Support NDIS goals and functional outcomes
• Provide AT assessments and recommendations
• Write clear, timely reports
• Manage a varied caseload and meet clinical targets
• Collaborate with families, referrers and other stakeholders
• Engage in regular supervision and professional development
