Job summary

Omnia Community Health in Victoria is seeking an Occupational Therapist to join the Adult Allied Health Team. The role offers 0.6–1.0 EFT with options for part-time to full-time work, delivering evidence-based OT across home, community and telehealth settings.

You will assess and treat adults with ageing, disability and chronic conditions, prescribing equipment and home modifications to enhance independence and safety.

Qualifications

• A minimum of three years of clinical experience as an Occupational Therapist.
• Experience assessing and managing adults with complex and chronic conditions.
• Demonstrated experience prescribing equipment, assistive technology and complex home modifications.
• Strong written and verbal communication skills, including report writing for SWEP, NDIS and SaH funding.

About The Role

This role is suitable to those looking at working 3 days (0.6EFT) per week, up to full-time (1.0 EFT).

As part of our Adult Allied Health Team, you will work withadults experiencing ageing, disability and chronic health conditions,delivering evidence-based Occupational Therapy services across home, communityand telehealth settings.

In this role, you will conduct comprehensive assessments and deliver evidence-based interventions for clients with complex and diverse needs. You will recommend and prescribe assistive technology, equipment, and home modifications that support independence, safety, and quality of life.

Working across a range of funding streams and programs, including NDIS, Support at Home, CHSP, HACC, and Community Health, you will collaborate closely with a multidisciplinary team to achieve positive outcomes for clients.

The role also provides opportunities to supervise and mentor students and early career clinicians, while contributing to quality improvement initiatives, service development activities, and the implementation of innovative models of care that enhance service delivery and client experience.

About You

You have:

• A minimum of three years of clinical experience as an Occupational Therapist.
• Experience assessing and managing adults with complex and chronic conditions.
• Demonstrated experience prescribing equipment, assistive technology and complex home modifications.
• A high level or written and verbal communication skills, including clinical report writing for the submission of reports and funding applications with Statewide Equipment Program (SWEP), NDIS and SaH.
