Austin Health is seeking an experienced Occupational Therapist to join the Health Independence Program (HIP) Fast Track team on a permanent part-time basis. You will work closely with Complex Care Managers and Physiotherapists to ensure clients are supported at home, assessing needs and arranging ongoing equipment and services.

HIP is part of the Continuing Care Division at Austin Health and provides a range of multi-disciplinary supports and services for clients to transition safety home from hospital to the community. We support people with health problems and other complex conditions to optimise their function and self-management skills so that they can live independently and well in their homes and community.
As the Grade 2 HIP Fast Track Care Management Occupational Therapist, you will enjoy the support of the multi-disciplinary team as well as other Occupational Therapists working in HIP.
As one of the two Occupational Therapists in the team, you will work closely with the Complex Care Managers and Physiotherapists to ensure that our clients are well supported at home. The Occupational Therapist is an integral member of the team, assessing clients and arranging ongoing equipment and service needs.
The Occupational Therapist will have ongoing support from the Complex Care Managers, a Geriatric Fellow and the Central Intake and Fast Track Care Management Team Leader.
