Job summary

Queensland Health's Transition Care Program in Cairns is seeking an Occupational Therapist (Case Manager) to support older adults transitioning from hospital to community. You will deliver high-quality, person-centred occupational therapy and case management within a multidisciplinary team.

You will work with older people, their carers, partners and families, providing consistent advice and coordinating care in Far North Queensland.

• Provide consistent, safe occupational therapy and case management services within a multidisciplinary team.
• Infrequent need for direct clinical practice supervision.
• Provide general clinical advice to managers and stakeholders across Far North Queensland.

• Provide general clinical advice to professional and operational supervisors and other stakeholders including non-government organisations and remote community agencies within Far North Queensland, regarding service delivery and case management on a regular basis and with infrequent need for supervision.

In this rewarding role, you will deliver high-quality, person-centred occupational therapy services that support older adults, along with their carers, partners, and families, as they transition from acute and sub-acute care back into the community.

About us:

The Transition Care Team provides a quality low intensity allied health intervention, nursing and case management service, to community-based clients post-discharge from acute and sub-acute hospital settings. Clients will have been recently assessed as eligible for residential care and appropriate for Transition

In this position you will…..
