Job summary

The Cairns and Hinterland Hospital and Health Service is seeking an experienced Occupational Therapist (Case Manager) to join the Transition Care Team in Cairns, QLD. You will deliver occupational therapy and case management as part of a multidisciplinary team to support community clients post-discharge.

You will work with diverse clients across Far North Queensland, providing clinical guidance to service managers and stakeholders while maintaining a flexible, patient-centered approach.

Qualifications

• Provide consistent, safe and efficient occupational therapy and case management services, within a multidisciplinary team, with infrequent need for direct clinical practice supervision.
• Demonstrate resilience and problem-solving abilities while working in varied and demanding caseload.

Responsibilities

• Provision of general clinical advice to professional and operational supervisors, relevant service managers and other stakeholders including non-government organisations and remote community agencies within Far North Queensland, regarding service delivery and case management on a regular basis and with infrequent need for supervision.
