You will assess and intervene with adults 18+, using assistive technology and home modifications, work with families and multidisciplinary teams, and contribute to service improvement and professional growth.

Job description

Join a dynamic and highly regarded Community Care Occupational Therapy team within Community Health and Integrated Care (CHIC), where you'll make a meaningful difference in the lives of adults living across the ACT. Working in community health centres and clients' homes, you'll support people with a diverse range of neurological, orthopaedic, chronic and complex health conditions to maximise independence, safety and participation in everyday life. As an experienced Occupational Therapist, you will provide high-quality assessment and intervention for adults aged 18 years and over, using assistive technology, home modifications and self-management approaches. Managing a varied and complex caseload, you will work collaboratively with clients, families, carers and interdisciplinary teams to develop client-centred solutions that enhance independence and wellbeing. You will also contribute to service innovation, quality improvement, education and mentoring, helping to strengthen and advance community-based care.
