Job summary

Calvary is seeking an experienced nurse leader to manage a multidisciplinary team in the Transitional Aged Care Program at Kogarah. This permanent full-time role focuses on coordinating care, improving patient flow and outcomes in a supportive environment.

You will influence clinical practice, foster change, and uphold quality and safety standards while supporting staff development and collaboration within community aged care.

Qualifications

• Registered with AHPRA as a Registered Nurse with a management qualification or equivalent experience.
• Current NSW Driver’s Licence and willingness to travel as required.
• Proven clinical leadership and team management in community aged care.
• Ability to analyse complex information to inform decision-making.
• Experience in quality and safety processes and data interpretation.
• Experience leading clinical practice change with strong emotional intelligence.

About the role

This permanent full time role is based at Kogarah is a newly created position within the Transitional Aged Care Program. This is a Monday to Friday role with standard business hours.

Your main duties will include:

• Provide operational management of a multidisciplinary team of approximately 9.7FTE (Clinical Nurse Specialists, Registered Nurses, EEN, Administration Officers and TACP Assessor)
• Manage the patient flow and care coordination through the TACP service to optimise access to the service, achieve great patient experience and outcomes and maximise occupancy.
• Drive quality improvement and clinical service standards whilst building the capability of the team to do the same

About you

• Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse and relevant management tertiary qualification or equivalent work experience, or a combination of study and work experience with a demonstrated commitment to professional development
• Current NSW Driver’s License and willingness to travel as required in role.
• Proven clinical leadership and team management skills, with a demonstrated commitment to delivering excellence in community aged care through person-centred care
• Demonstrated ability to use analyse and interpret complex verbal, written and numerical information, and draw logical conclusions to inform decision-making.
• Demonstrated experience in the management of quality and safety processes, including the ability to analyse patterns and trends within new or unique sets of data.
• Demonstrated experience leading and implementing change in clinical practice within a person-centred framework, with strong emotional intelligence and the ability to positively influence and engage others.
