Queensland Health’s Metro North Health is seeking a Nurse Navigator – Outpatient Departments to lead coordination of complex patient journeys and provide expert clinical guidance.

You will work with consumers, families and clinical teams to improve access to outpatient services, drive system improvements and strengthen care coordination across the network.

This role offers opportunities to influence practice and deliver person-centred care in a dynamic hospital setting.

Competenze

• Provide advanced nursing leadership and care coordination for consumers with complex clinical needs accessing outpatient services.
• Coordinate consumer-centred care across the outpatient journey to ensure timely access and reduce fragmentation.
• Act as central point of communication between consumers, families, healthcare providers and community services to enable integrated care delivery.
• Apply advanced clinical knowledge and system expertise to support vulnerable populations, improve health literacy and promote self-management.
• Lead quality improvement initiatives, service development and evidence-based practice to enhance patient outcomes in outpatient care.
