The Royal Melbourne Hospital in Melbourne is recruiting a Social Worker for the Neuropsychiatry service. You will deliver high-level clinical support to patients and carers, working within an inpatient multidisciplinary team to promote person-centred care and effective discharge planning.

Responsabilités

• Provide assessment, intervention and support to patients with complex neuropsychiatric presentations and their carers.
• Work closely with inpatient and multidisciplinary teams to support person-centred care and discharge planning.
• Advocate for patients and connect them with appropriate health, community and social supports.
• Build strong partnerships with patients, carers and internal and external service providers.
