Job summary

NSW Health is seeking a Nurse Manager Grade 4 to provide strategic, professional and operational leadership for Community Aged Care Services across the Illawarra Shoalhaven Local Health District. You will oversee Transitional Care, Aged Care Assessment Services and Commonwealth Home Support Program services, lead multidisciplinary teams, and drive governance, performance and continuous improvement.

The Network Community Aged Care Manager provides strategic, professional and operational leadership for Community Aged Care Services across the Illawarra Shoalhaven Local Health District. In partnership with the Director Out of Hospital Care, the position is responsible for the delivery, performance and continuous improvement of District-wide aged care services, including Transitional Care, Aged Care Assessment Services and Commonwealth Home Support Program services.

The role leads multidisciplinary teams to ensure effective clinical governance, workforce management, service integration, financial performance and compliance with State and Commonwealth requirements. Through leadership of innovation, reform and partnership activities, the position supports contemporary models of care and improved outcomes for older people accessing community-based and out-of-hospital services.
