Wholesome Solutions Pty Ltd in Caboolture, QLD is seeking a dedicated NDIS Support Coordinator on a part-time basis. You will coordinate and facilitate NDIS plans, liaise with providers, and advocate for clients to ensure tailored supports.

Qualifications

• Experience is essential — ideally in Support Coordination within the NDIS sector.
• Ability to understand NDIS plans and navigate the scheme on clients' behalf.
• Current NDIS Worker Screening Check (NDISWC) and Working with Children Check preferred or required.

Responsibilities

• Coordinate and facilitate the implementation of NDIS plans for clients.
• Liaise with service providers, government agencies and other stakeholders to coordinate supports.
• Advocate on behalf of clients to secure appropriate services.
• Review and update client NDIS plans and budgets as needs change.
• Maintain detailed records supporting progress and service delivery.

Wholesome Solutions Pty Ltdis seeking a dedicated and experienced NDIS Support Coordinatorto join our team. This is a part time position, with the opportunity to move into full-time for the right candidate. Offering the chance to make a meaningful impact in the lives of individuals living with disabilities, you will play a vital role in navigating the NDIS system and ensuring our clients receive the tailored support they need to thrive.

What you'll be doing

• Coordinate and facilitate the implementation of NDIS plans for clients, ensuring their involvement every step of the way to meet their goals and needs
• Liaise with service providers, government agencies and other stakeholders to coordinate and monitor the delivery of supports
• Assist clients in understanding their NDIS plans and the available support services
• Advocate on behalf of clients to secure appropriate and effective support services
• Regularly review and update client NDIS plans and budgets to reflect changing needs and circumstances
• Maintain detailed records and documentation to support client progress and service delivery
• Collaborate with the broader Wholesome Solutions team to provide holistic care and support.

What we're looking for

• Experience is essential — ideally in Support Coordination. Other relevant roles within the NDIS sector may also be considered.
• Demonstrated understanding of the NDIS and experience navigating the scheme on behalf of clients
• Relevant diploma / tertiary qualification in a human services or allied health discipline is advantageous
• Bring your current caseload - we welcome support coordinators who would like to bring along participants, whilst having the capacity to take on more as referrals come through.
• Excellent communication and interpersonal skills, with the ability to effectively liaise with a range of stakeholders
• Strong problem-solving and critical thinking skills to identify and address client needs
• Commitment to providing person-centred, empowering support to clients with diverse needs
• Willingness to travel within the local area to meet with clients as required
• Current driver's licence and access to reliable transportation
• Current NDISWC and Working with Children Check
