Job summary

Advantage Healthcare Support Nursing Agency in Dee Why, NSW, offers a full‑time Community Support Worker role based in Dee Why with travel to participant locations across the Northern Beaches. Qualifications

At least 1 year full-time experience in relevant work.
Strong written and verbal communication skills including accurate case notes.

Responsibilities

Assess participant needs and develop, implement and review individualized support plans.
Coordinate health, community and NDIS-funded services and liaise with external providers.
Support participants to build independence and achieve goals in their plans.
Monitor progress, wellbeing and changes in support needs; escalate concerns when needed.
Maintain accurate client notes, progress reports and service records.
Participate in case reviews, team meetings and training.
