Job summary

ATSICHS Brisbane invites applications for an NDIS Access Support Officer to guide clients through Access Requests, eligibility assessments, and documentation across Brisbane and Logan regions. You will collaborate with GPs, therapists, and external providers, maintain accurate case notes, and monitor progress while travelling between community sites to support clients and families.

The role requires a health or community services qualification and a commitment to culturally safe, person-centred

Qualifications

• Intake, screening, and assessment reviews
• Accurate case notes and client records
• Professional judgement in goal setting and service navigation
• Ability to work with minimal direction and in a multidisciplinary team
• Travel between work sites to communities and health locations

Responsibilities

• Support prospective clients through the NDIS Access Request process
• Assist clients to understand NDIS eligibility requirements and pathways
• Coordinate with health professionals and external service providers to obtain documentation
• Collaborate with GPs, therapists, and other practitioners to demonstrate needs and supports
• Facilitate completion and submission of Access Requests and related documentation
• Support clients through NDIA reviews and appeals where required
• Provide information on NDIS and available supports to clients and families
• Maintain timely and detailed participant records in accordance with policies
• Monitor application progress and communicate with clients throughout the process
• Refer ineligible individuals to appropriate supports and services
• Report on program activities and outcomes
• Assist in planning and delivery of NDIS information and community sessions
• Contribute to resource development on NDIS access and eligibility
• Maintain quality assurance and compliance with procedures
• Stay informed of NDIS legislation and guidelines
• Travel regularly between work sites and in the community
