Job summary

Lifecare Homecare is seeking a Physiotherapist to deliver high-quality in-home care across Western Australia. You will assess, plan, implement, and evaluate treatments in a multi-disciplinary setting, focusing on the individual needs of clients in their own homes.

Applicants must hold a Physiotherapy degree with General AHPRA registration and have full working rights in Australia. You will collaborate with other health professionals and prepare reports aligned with funding bodies such as NDIS,

Qualifications

• Physiotherapy degree with General AHPRA registration.
• Full working rights in Australia.
• Ability to work within a multi-disciplinary team in the community.

Responsibilities

• Provide home-based physiotherapy assessments and interventions.
• Develop and implement treatment plans with progress reports.
• Write initial, progress and final reports for funding bodies (NDIS, DVA, Medicare).
• Liaise with GPs and other health professionals to coordinate care.
• Maintain relationships with case managers and referrers.
