Community Living Supports (CLS) is a state-wide psychosocial support program for people with complex mental health needs, with the focus on supporting consumers to live well in their community with choice, dignity and self-determination. CLS works alongside consumers to build on their strengths and aspirations, offering practical support that promotes recovery, confidence, independence and connection to community.
The CLS Mental Health Support Worker delivers flexible, person-led support in line with the consumer's Individual Service Plan, collaborating with their chosen supports (including LHDs and community services) to ensure coordinated and culturally respectful care. Support is reviewed and adjusted as needs and recovery goals evolve, with family or carers involved, where consent is given.
