Job summary

Open Minds is seeking a compassionate Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.

You will work with a multidisciplinary team, support smooth transitions between services, drive quality improvement and contribute to positive client outcomes through ethical, collaborative practice.

Qualifications

• Experience using trauma-informed, recovery-focused approaches with people who have severe mental health challenges.
• Experience working with diverse cultures, especially First Nations people.
• Strong teamwork and collaboration skills with problem-solving and organisational abilities.
• Current Mental Health Accreditation with AHPRA or eligibility for AASW Membership.
• Tertiary qualification in a relevant mental health field.

Responsibilities

• Conduct clinical assessments and develop tailored treatment plans.
• Deliver evidence-based clinical and psychosocial interventions.
• Autonomously manage caseload and conduct risk assessments with management plans.

Responsibilities

Requirements
