Job summary

St Jude’s Health Care Group is seeking a Lifestyle Coordinator to develop and oversee a resident-focused Lifestyle program that reflects individual preferences and abilities. The role involves mentoring staff and delivering programs that foster social activity and emotional connection for residents.

The successful candidate will liaise with health professionals and family members, build rapport with clients, and ensure plans align with policy and risk management.

Qualifications

• Ability to relate to elderly clients with empathy and build rapport with families and health professionals.
• Experience in aged care including assessment and equipment prescription where applicable.
• Experience in development and delivery of therapy-based programs for residents.
• Demonstrates commitment to teamwork and positive collaboration.
• Excellent interpersonal, communication and time management skills.
• Knowledge of community-based programs and flexible approach to activities.

Responsibilities

• Establish positive communication with residents and next of kin to develop activity assessments and care plans.
• Develop, implement and evaluate individual social and leisure care plans; mentor the team.
• Identify entertainment options and incorporate them into a monthly program; promote resident engagement.
• Build positive relationships with staff, nurses and volunteers to support holistic wellbeing.
• Ensure lifestyle coordination aligns with organisational policies and manages risks.
