Job summary

Rockpool Residential Aged Care in Queensland is seeking a Lifestyle Coordinator to design and coordinate monthly lifestyle programs aligned with governance, the Strengthened Aged Care Quality Standards and person-centred care. You will assess residents, develop individualised lifestyle care plans, and coach Lifestyle Officers within care teams.

Qualifications

Demonstrated experience designing and delivering lifestyle programs for older people, including residents living with dementia.
Solid knowledge of Strengthened Aged Care Quality Standards and person-centred assessment and care planning.
Strong documentation and computer skills.

Responsibilities

Design and maintain a monthly lifestyle calendar of group and individual activities reflecting residents' interests.
Complete assessments and social profiles; develop individualised lifestyle care plans.
Coach Lifestyle Officers and partner with leaders to ensure high-quality engagement seven days.
Build and maintain community partnerships and intergenerational programs.
Plan events and culturally significant days to bring the home together.
Collaborate with clinical teams to align lifestyle with wellness programs.
Identify residents at risk of social isolation; ensure meaningful one-to-one engagement.
Ensure activities are inclusive for dementia and mobility or sensory needs.
Manage the lifestyle budget, resources and supplier relationships.
