Job summary

Calvary Brighton-le-sands is seeking a dedicated Leisure & Activities Coordinator to support the Home Manager and Clinical Leadership in designing and delivering individual and group activities for residents. The role requires understanding residents’ identities, cultures and life experiences, and works within an aged care setting.

The successful candidate will plan activities, liaise with families, and document assessments and care plans in accordance with legislative and quality requirements.

Qualifications

• Certificate IV Leisure & Lifestyle or other relevant qualification.
• Current First Aid Certificate.
• Experience in planning, implementation and evaluation of activities for frail people with cognitive impairment and other disabilities.
• Ability to document assessments, care plans and progress notes to meet legislative, funding and quality improvement requirements.

Responsibilities

• Care planning, design and develop group and 1:1 care programs for residents that takes into account their individual lifestyle, hobbies and spiritual needs.
• Design and plan individual activities aligned to resident interests and needs that can be carried out by the care team.
• Liaise with families to assess needs and compile appropriate activity assessments and care plans.
