Job summary

NSW Health at Westmead Hospital is seeking a Clinical Nurse Consultant Grade 2: Blood Transplant Post-Transplant Coordinator to lead advanced post-transplant care from Day 100 onwards, coordinating complex outpatient and inpatient pathways.

You will provide expert clinical leadership, education for patients and staff, and drive research, quality improvement and service development within a specialist Blood Transplant and Cell Therapy service.

Qualifications

• Minimum five years full-time post registration experience.
• At least three years in specialty field.
• Experience developing, implementing and reviewing policy, procedure and guidelines.
• Demonstrated effective communication and interpersonal skills in a multidisciplinary context.
• Ability to work effectively independently and build professional relationships across multiple disciplines.
• Demonstrated skills in providing education for patients, carers and staff.

Responsibilities

• Provide advanced clinical expertise and autonomous care coordination for post-transplant patients.
• Coordinate complex clinical care across outpatient and inpatient settings.
• Provide expert consultancy and clinical leadership in post-Transplant care.
• Lead education for patients, carers and healthcare professionals.
• Drive evidence-based practice, research and quality improvement initiatives.
