Job summary

Barwon Health is seeking a Lead Aged Care On-Site Pharmacist (Grade 3) to lead high-quality pharmacy services across residential aged care facilities and care transitions. You will ensure safe medication use, drive continuity of medication management and support governance through reporting to the Medicines Advisory Committee.

You will contribute to education of interns, undertake clinical pharmacy research and present at conferences, while expanding skills across hospital pharmacy settings

Qualifications

• Current registration as a Pharmacist with AHPRA.
• Credentialed for APC-accredited aged care on-site pharmacist training program.
• Credentialed for Medication Management Reviews (MMR).
• Excellent leadership, communication and stakeholder engagement skills.

Responsibilities

• Contribute to RACF clinical governance and encourage a culture of continuous quality improvement to ensure safe and quality care for residents.
• Supporting best practice templates, guidelines, processes and protocols to drive system-level improvements.
• Providing specialist pharmacist representation as a member of the Aged Care Incident Review Committee and of the Medication Safety Committee.
