Job summary

Barwon Health is seeking a Lead Aged Care On-Site Pharmacist Grade 3 to oversee high-quality pharmacy services across residential aged care facilities and transitions of care. You will lead medication management, ensure safe and efficient use of medicines, and contribute to governance and education within Barwon Health.

The role offers a full-time position (80 hours per fortnight) with opportunities for professional development and involvement in system-wide quality improvement initiatives.

Qualifications

• Current Pharmacist registration with AHPRA.
• APC-accredited aged care on-site pharmacist training.
• Credentialed for Medication Management Reviews (MMR).
• Strong leadership and stakeholder engagement skills.

About The Role

As the Lead Aged Care On-Site Pharmacist Grade 3, you will lead the delivery of high-quality pharmacy services across Barwon Health’s residential aged care facilities and care transitions. You will support safe, appropriate and efficient medication use in line with the objectives of the Aged Care On-site Pharmacist Measure and Barwon Health’s strategic priorities.

Key responsibilities include providing continuity of medication management, resolving medication-related issues and supporting effective communication during transitions of care. You will also lead whole-of-facility quality use of medicines activities and contribute to medicines governance through reporting and advice to the Medicines Advisory Committee.

This role provides opportunities to contribute to the education and development of pharmacy interns and students, undertake clinical pharmacy research and present at professional conferences. You will also have the opportunity to broaden and develop your skills across a range of hospital pharmacy settings within Barwon Health.

Key Responsibilities

• contribute to RACF clinical governance and encouraging a culture of continuous quality improvement to ensure safe and quality care for residents


• supporting best practice templates/guidelines, processes and protocols to drive system level improvements


• providing specialist pharmacist representation as a member of the Aged Care Incident Review Committee and of the Medication Safety Committee

What We're Looking For

• Current registration as a Pharmacist with AHPRA and significant hospital pharmacy experience.


• Credentialed for the Australian Pharmacy Council (APC)-accredited aged care on-site pharmacist training program


• Credentialed for Medication Management Reviews (MMR)


• Excellent leadership, communication and stakeholder engagement skills, with the ability to influence organisation-wide practice.
