Job summary

Berry Street in Ballarat is seeking an Intake and Assessment Worker to support the timely intake and assessment of students referred to the Navigator Program. You will engage with young people and families using assertive outreach, complete initial assessments, and coordinate the documentation for triage and intake, working closely with the Navigator Team Leader.

Job description

The Navigator Program is funded by the Department of Education and supports young people aged 12 to 17 who are disengaged, or at risk of disengaging, from education.

The program works closely with young people, their families, schools and broader support networks to identify barriers to participation and develop tailored strategies to support re-engagement with education or training.

This includes strengthening connections with relevant services and professionals to improve educational, wellbeing and long-term outcomes for young people.

• Ballarat location, Wadawurrung Country
• Part time (0.4 FTE), 12 month Fixed Term (funding)
• $86K pa (pro rata) + Super & Salary Packaging

Your Opportunity

The Intake and Assessment Worker supports the timely intake and assessment of students referred to the Navigator Program.

In this role, you will use assertive outreach strategies to engage positively with young people and their families, undertake initial assessments and coordinate the documentation required to commence the program’s triage and intake processes.

Key Responsibilities

• Use assertive outreach strategies to engage students and families referred to the Navigator Program by the Department of Education.
• Undertake initial assessments and support students and families through the intake process.
• Manage the suite of intake documentation required to commence program triage.
• Maintain accurate and timely information within the Navigator Data Management System in accordance with program guidelines.
• Work closely with the Navigator Team Leader and broader program team.
• Provide information and updates to support program governance and information-sharing requirements.
• Build positive working relationships with schools, education providers, community organisations and support services.

About You

You will have demonstrated experience providing outreach support or case management to young people aged 12 to 17 who are disengaged from education, socially isolated or at risk of disengagement.

You Will Also Have

• The ability to build rapport with young people and families who may be reluctant to engage.
• Strong communication, assessment and stakeholder engagement skills.
• The ability to negotiate and advocate effectively with local services, education providers and training organisations.
• Strong organisational skills and the ability to maintain accurate program documentation.
• A bachelor's degree in Education, Youth Work, Social Work, Community Development or a related discipline.
