Job summary

Metro South Health Service in Brisbane is seeking an Aboriginal and Torres Strait Islander Health Worker - Identified to deliver culturally safe primary healthcare to Aboriginal and Torres Strait Islander peoples and their families.

The role focuses on health assessments, immunisations, health education, building community partnerships, and supporting referrals within scope of practice. You will work autonomously within a multidisciplinary team and contribute to continuous service improvement.

Qualifications

• You have a strong understanding of Aboriginal and Torres Strait Islander cultures and communities.
• Clinical skills and knowledge for primary healthcare delivery in Aboriginal and Torres Strait Islander communities.
• Ability to communicate effectively with clients, families and stakeholders.
• Capacity to work with autonomy, exercise judgement and escalate risks appropriately.
• Commitment to culturally safe, high-quality care within a multidisciplinary team.

Responsibilities

• Deliver culturally appropriate primary healthcare services including health assessments and immunisations.
• Facilitate health education and promotion for diverse community groups.
• Build and maintain relationships with communities and partner organisations.
• Advocate for individuals, helping navigate services and referrals.
• Identify community health priorities and support continuous service improvement.
