Job summary

Ngwala Willumbong is seeking a Complex Case Manager to deliver intensive, culturally safe case management for Aboriginal and Torres Strait Islander community members with complex AOD, homelessness and psychosocial needs in Victoria.

The role works with NRCH and MSIR to provide outreach, in-reach support, risk assessment, and integrated care planning across AOD, SEWB and related services, with strong emphasis on trauma-informed, strengths-based practice.

Qualifications

• Experience in AOD, mental health or homelessness services.
• Experience providing culturally safe support to Aboriginal people.
• Understanding of harm minimisation, trauma-informed care, and strengths-based practice.
• Ability to work collaboratively with multiple service partners.

Responsibilities

• Provide intensive, culturally safe case management tailored to Aboriginal community members with complex needs.
• Undertake comprehensive assessments, case planning, review and goal setting.
• Provide in-reach and outreach support with NRCH, MSIR, and related services.
